By submitting this reservation, you are purchasing a Nourished by the Moon session (“Session”) with Nourished by ME LLC (“Company”). You agree to the following terms and conditions. 

What’s Included: Prior to your Session, you agree to submit responses to a questionnaire. We’ll use your responses to create your Nourished by the Moon session information and report. During your Session, we will review your responses. 

Information You’ll Need: To answer the questionnaire, you will need to provide us with the exact date, time, and place of your birth. These are important pieces of information that allows us to create your personalized report. Please ensure that you have entered the correct information as the time will influence our analysis. 

Additional Sessions: You will have the ability to add extra coaching sessions. During these additional sessions, we will assist you with implementing the information you learned during your Session and from your Nourished by the Moon report. 

Payment: All payments are due at the time you schedule your Session. 

Refunds: All sales are final. No refunds will be granted if you do not participate or attend.
